Below is a list of documents and examples of each type of document to bring when applying for AND, OAP, QMB, SLIM-B, and QI-1. Bring as many documents as possible. If you do not have all of the required paperwork, you can provide it after the first interview.

- Baptismal certificate
- Document at least 10 years old with birth date
- Birth certificate

- Social Security Card
- Any photo ID with your address
- Rent receipt
Payroll stubs
Verification of social security and SSI
Proof of other income or checks you receive (child support, alimony, self-employment, financial aid)
- Bank statements
- Value of motor vehicles (if more than one vehicle)
- Tax statement for real estate property
- Life insurance letter showing face and cash value
- Burial funds or policies
- IRA/stocks/bonds
