The third year and sixth year of self-certification are allowed for a permanent disability, by completing the Persons with Disability License Plate Renewal Notice. On and after January 1, 2014, the DR 2219 Persons with Disabilities Parking Privileges Application must be completed in its entirety including the verification by a professional for all license plates and permanent (3-year) placards being renewed in their ninth certification year.
